    Good afternoon,
     
     
    As you know, vaccine eligibility includes teens who are at least 16 years old. If you are interested in your child receiving a COVID-19 vaccination through Overlook Medical Center/Atlantic Health System, please use the link below to complete an associated form with his/her information. Kindly complete a different form for each of your children who you are interested in receiving the vaccine by the end of the day next Wednesday (the 28th).
